Preamble

EpiGenetics USA operates as a functional wellness consulting practice under the clinical direction of Dr. Patrick Lambert, N.D., D.C. (Ret.), F.A.S.B.E., and Dr. Troy DanDurand, Ph.D., M.B.A., M.M.Sc. We are committed to transparency, informed consent, and your empowerment as a client.

This Client Bill of Rights establishes the foundation of our professional relationship. By signing below, you acknowledge that you have read, understood, and agree to these terms.

Your Rights as a Client

Right to Informed Consent. You have the right to receive a clear explanation of any assessment, recommendation, or program before you agree to participate. No service will be rendered without your express consent.

Right to Transparency. You have the right to know the scope, cost, and expected duration of any recommended pathway, product, or service before making a financial commitment.

Right to Privacy. Your personal health information, assessment results, and consultation records are confidential. We will never share your information with third parties without your written authorization.

Right to Ask Questions. You have the right to ask questions at any point during the assessment or consultation process and to receive clear, honest answers free from pressure or coercion.

Right to Refuse. You have the right to decline or discontinue any service, product, or recommendation at any time without penalty.

Right to Qualified Practitioners. You have the right to be served by credentialed practitioners. Our Clinical Director holds a Fellowship in Applied Spinal Biomechanical Engineering (F.A.S.B.E.) from the American Academy of Applied Spinal Biomechanical Engineering.

Right to a Second Opinion. We encourage you to seek additional medical counsel at any time. Our recommendations complement — never replace — your relationship with your primary care physician.

Right to Your Records. You have the right to request a copy of your assessment results, Holistic Intelligence Dossier, and any documentation produced during your engagement with our practice.

Scope of Services — What We Do

We provide functional wellness consulting, nutritional guidance, health education, lifestyle coaching, and technology-assisted wellness assessments including the Medical Symptom Questionnaire (MSQ), Discovery Deposition, and Holistic Intelligence Dossier.

Products and technologies offered may include BEMER physical vascular therapy devices, NutriDyn professional-grade supplements, Dr. Pat Luse’s 7 Systems Plan educational curriculum, and personalized wellness pathways.

Scope of Services — What We Do Not Do

Financial Commitment

All fees are disclosed in advance. No hidden charges. Standard service fees:

• Holistic Health Intake Form (HHIF): $349
• Holistic Intelligence Discovery Dossier (HIDD): $549
• 7 Systems Plan + Coaching: $897 + ~$150/month supplements
• BEMER Devices: $3,900 – $5,890 depending on configuration

EpiGenetics USA operates in compliance with Minnesota Statute §146A governing unlicensed complementary and alternative health care practices. In accordance with §146A.11, this document serves as your written acknowledgment that you understand the nature and scope of our services before they are rendered.
